Changes to the prescribing information for alteplase Activase rt-PA pertaining to contraindications to the drug's use in acute ischemic stroke AIS recently flew in under the radar. The consequences that used to be "significant disability or death" are now simply "situations in which the risk of bleeding is greater than the potential benefit. At this point, it is unclear if or when practice guidlines will be updated based on the new prescribing information.

Tissue plasminogen activator tPA, Alteplase catalyzes the conversion of plasminogen to plasmin, the major enzyme responsible for clot breakdown. Its fibrinolysis properties makes it clinically useful for treatment of treatment of myocardial infarction with ST-elevation STEMI , acute ischemic emboilc or thrombotic stroke AIS , acute massive pulmonary embolism, and central venous access devices CVAD. Alteplase is produced by Genentech, and is manufactured using recombinant biotechnology techniques. Other r-tPA medications are Reteplase and Tenecteplase. Unfortunately these medications are not used daily, and often only in emergent situations.

Alteplase, recombinant Activase is a fibrinolytic enzyme used to dissolve blood clots in patients with acute pulmonary embolism PE , acute myocardial infarction MI , and acute ischemic stroke. Under the trade name Cathflo Activase, it's also indicated to treat central venous catheter occlusion.
